Is Your Form 498 Pending?

It was brought to my attention last week, that several school districts/libraries in Georgia have Form 498s that have been pending (“certified” but not “approved”) for a lengthy amount of time. Remember, the Form 498 collects your banking information and is a new form required for all applicants using the BEAR invoicing method. Most likely, the reason your Form 498 is pending is because supporting documentation was never provided. Follow these steps to complete the 498 process:
- Upload your “supporting documents” - What is required for Bank Validation? A clear image of a voided check, or a statement from your financial institution that includes the bank name and your account number. Please submit the supporting documents by clicking HERE.
- Send an email to FINOPS-Processing@usac.org notifying them that your supporting documents have been uploaded. Include your 498 ID (443XXXXXX) in the email.
According to the Billings, Collections and Disbursements team at USAC, once you have completed these two steps, your Form 498 should be approved within 24-48 hours and you will then be able to file BEARs, as needed. Please let me know if you have any issues with these steps or if your form is not approved quickly. (This email was sent directly to all affected applicants)
