FY2017 Form 479 (CIPA form)

As consortium lead for GaDOE’s statewide K-12 Network, the Georgia Board of Regents (BOR) must collect FCC Form 479s for all consortium members for E-rate purposes. The FCC Form 479 is collected to provide notification that your district complies with the Children’s Internet Protection Act (CIPA). Click here to review the CIPA requirements and steps on how to document compliance. Please complete an FY2017 FCC Form 479 and email to Candice.Hall@usg.edu by March 3, 2017. **Note that only (a.) in Block 2 should be checked. Here is an EXAMPLE FCC Form 479 for guidance.
Some of you may remember having to complete a Letter of Agency (LOA) as well. The LOA grants the BOR the authority to apply for discounted telecommunications and internet services (GaDOE K-12 Network connection ONLY) on behalf of your district. A new LOA is not needed this year, as the one completed in 2015 is effective until 2020.
